- Confirmation Registration Form
  
  This form is for students beginning their preparation for the Sacrament of Confirmation. Students are able to begin Confirmation I in the fall of their 9th grade year. Preparation for Confirmation is two years.
  
  The cost of enrollment is $125 which includes books, materials, and a retreat.
  (Students returning for Confirmation II do not need to register again.)
  
  Multiple Student Discount
  OLL does not charge more than $200 in Faith Formation fees for the Confirmation I & II program. If multiple students are enrolling from your immediate family this year, it will only cost $75 to enroll the second child, and any additional children may be enrolled for free.
  
  Baptismal Certificate
  A copy of your child's baptismal certificate should be submitted to the parish office or emailed to Austin Faur shortly after you register or before September 14. (afaur@ourladyoflourdescc.org)

  The Catechism of the Catholic Church says, “The Sunday Eucharist is the foundation and confirmation of all Christian practice.”
  
  It is of the highest importance that all Confirmation candidates attend Mass every Sunday throughout the entire year and on Holy Days of Obligation.

- Intellectual and Spiritual Formation Expectations
  "Education in the faith by the parents should begin in the child's earliest years. This already happens when family members help one another to grow in faith by the witness of a Christian life in keeping with the Gospel. Family catechesis precedes, accompanies, and enriches other forms of instruction in the faith.
  
  Parents have the mission of teaching their children to pray and to discover their vocation as children of God. The parish is the Eucharistic community and the heart of the liturgical life of Christian families; it is a privileged place for the catechesis of children and parents."

  Students are expected to attend faith formation. While we understand conflicts will arise, it is crucial that students have adequate time in faith formation. If a student is absent for a class, it is still expected that the family complete the online session for that lesson.
